When you have an online business. What credit card transaction do you carry. For instance, "Pay pal." Any better ones?

Do you carry a separate credit card machine at home/office for faxed orders or trade shows?
Do you know of any companies that sell the machines reasonably?
My friend looked into it at one time and said it was $5000 start up and charged 2% for each transaction. Is this normal?
How do you charge for international orders. I hear that international credit cards and transactions (shipping) are a pain.
